Short answers to the questions men ask before and after prostate surgery. Each one links to the longer version.
How do you stop urine leakage after prostate surgery?
By training the external sphincter, the part of the pelvic floor you can control. Learn to contract it correctly, squeeze it right before coughing, standing, or lifting, build both slow holds and quick contractions, and practice in sitting, standing, and walking, not only lying down.
Why do men leak after prostate removal?
Two sphincters keep a man dry. The internal one sits where the prostate was and is damaged or removed in surgery. The external one, part of the pelvic floor, has to do the job alone and needs training to catch up.
Should I drink less water to leak less after prostate surgery?
No. Drinking less concentrates the urine, which irritates the bladder and can increase urgency. Drink normally and cut back on caffeine and alcohol instead.
When should I start kegels after prostate surgery?
After the catheter is out and your surgeon has cleared you. Do not do pelvic floor exercises while the catheter is in.
How many kegels should a man do after prostatectomy?
Start small: a two to three second hold, ten seconds of rest, five repetitions, three times a day. More is not better. A fatigued pelvic floor is a weaker one.
How do I know if I am doing kegels correctly?
You should feel a gentle squeeze and lift inside the pelvis while your belly and glutes stay relaxed and you keep breathing normally. If you are holding your breath, tensing your abs, or pushing downward, it is not a correct contraction.

Should I do kegels before prostate surgery?
Training the pelvic floor before surgery is associated with fewer pads sooner, and most men are never told to do it. Ask your surgeon whether there is any reason you should not, then learn to find the muscle, to squeeze before pressure, and to relax fully.
What should I buy before a prostatectomy?
Male incontinence guards and pull-up style underwear, a waterproof mattress protector, loose dark clothing, a small toilet stool, flushable wipes, and the stool softener your surgical team recommends.
Do I keep doing pelvic floor exercises with a catheter in?
No. Stop when the catheter goes in and restart once it is out and your surgeon clears you.
How soon can I walk after prostate surgery?
Walking starts in the first week: short, slow walks a few times a day. Walking is the only exercise for roughly the first four weeks. Follow your surgeon's specific limits.
When can I lift weights after a prostatectomy?
As a general timeline, light weights under twenty pounds around weeks six to eight, increasing gradually from weeks eight to twelve, once your surgeon has cleared you. If an exercise makes you leak, the load is too much for now.
When can I run after prostate surgery?
Light jogging is usually introduced between weeks eight and twelve, after you can walk briskly for thirty minutes with very little leaking. Start with walk and jog intervals and go back to walking if you leak.

How long does the catheter stay in after prostate surgery?
Most men keep the catheter for seven to fourteen days. Your surgeon will give you the exact plan.
Should I do kegels while the catheter is in?
No. Wait until the catheter is out and your doctor has cleared you. During the catheter week the job is to walk, rest, and stay hydrated.
Is it normal to leak when the catheter comes out?
Yes. Most men leak at first and some leak a lot. How much you leak in the first days says very little about where you end up.
How long is recovery after prostate surgery?
Most of the recovery happens over about twelve weeks, in stages: the catheter week, the start of leaking, then steady improvement as the pelvic floor is trained. Some men recover faster and some take longer. Both are normal.
What is the fastest way to recover after prostatectomy?
Walk from the first week, start pelvic floor exercises once the catheter is out and your doctor clears you, do them correctly and every day, and return to activity in stages. Doing more repetitions than the plan calls for does not speed it up.
When should I see a pelvic floor physical therapist after prostate surgery?
If you see no improvement by week eight, if you cannot tell whether you are squeezing the right muscle, if leaking is getting worse, or if you have pelvic pain.
What are the best pads for men after prostate surgery?
Use products made for men. Thin male guards such as Depend Guards for Men or TENA Men work for light leaking, and pull-up style underwear such as Depend Real Fit for Men works for the heavy first weeks. Add a waterproof mattress protector for the night.
Can men use women's pads after prostate surgery?
Male guards are the better choice. They are shaped differently and catch leaks where men actually leak.
Should I drink less water to use fewer pads?
No. Drinking less concentrates the urine, which irritates the bladder and can increase urgency. Cut back on caffeine and alcohol instead.

Do I need physical therapy after prostate surgery?
Many men recover with a clear plan and daily pelvic floor work at home. See a pelvic floor physical therapist if leaking is not improving by week eight, if it is getting worse, if you cannot find the right muscle, or if you have pelvic pain.
How do I find a pelvic floor physical therapist who treats men?
Search the American Physical Therapy Association directory and filter for pelvic health, or ask your urologist for a referral. Call ahead and ask whether they treat men with incontinence after prostatectomy.
What does a pelvic floor physical therapist do for men after prostatectomy?
They assess muscle function, strength, and coordination, sometimes with biofeedback or an internal assessment, and build an exercise program for you.

How do men do kegel exercises?
Gently squeeze and lift the pelvic floor, the muscles you would use to stop the flow of urine or hold back gas. Hold two to three seconds, let go completely, and rest ten seconds. The belly and glutes stay relaxed and you keep breathing.
How many kegels should a man do a day?
A starting dose is five repetitions, three times a day, with a two to three second hold and ten seconds of rest. More is not better. The muscle tires like any other.
